Malcolm Crowe (Bruce Willis) è un riconosciuto e premiato psicologo infantile che vive a Philadelphia con la moglie Anna (Olivia Williams).

Un sera, tornando da una cena, trova in casa ad attenderlo Vincent Gray (Donnie Wahlberg) un suo ex paziente, ormai adulto, che minaccia di ucciderlo perché lo accusa di non averlo aiutato a guarire dalle sue allucinazioni. Gray gli spara e poi si suicida.

Qualche tempo dopo Crowe, sopravvissuto allo sparo, prende in cura Cole (Haley Joel Osment), un timido bambino di nove anni, che gli confessa di riuscire a vedere e parlare con le persone morte. Inizialmente lo psicologo è scettico, ma è deciso a trovare il modo per aiutare il bambino. Così lo convince che se ha quegli eccezionali poteri, deve considerarli un dono e quindi usarli per aiutare le anime che gli parlano a risolvere ciò che di incompiuto non gli lascia abbandonare la terra.

Così Cole entra in contatto con Kyra, una bambina che gli confessa di essere morta avvelenata di nascosto dalla madre che è affetta da un terribile disturbo mentale. La ragazzina ha una sorella e gli chiede aiuto per evitare che anche lei faccia la sua stessa fine. Cole forse ha trovato il modo di convivere con i fantasmi che vede e ad accettare il suo dono...
