Lt. Kinderman and Father Dyer cheer each other up on the anniversary of the death of their mutual friend, Father Damien Karras, by going to see "It's a Wonderful Life" at the local theater in Georgetown, near Washington D.C. But there's no cheering Kinderman while a particularly cruel and gruesome serial killer is at large. His murders, which involve torture, decapitation and the desecration of religious icons, is bad enough; but they also resemble those of the Gemini Killer, who has been dead for fifteen years.

A seemingly-endless series of grisly killings that bear the trademark of the mass murderer the Gemini Killer, terrorizes the district of Georgetown. To further complicate matters, although it's been 17 long years since the killer's execution and that fatal night of pure terror in L'esorcista (1973), skeptical police officer Lieutenant William F. Kinderman is still obsessed with solving the baffling case, as the death toll keeps rising. In the meantime, in the city's high-security psychiatric institution, a cryptic inmate who bears an uncanny resemblance to the late Father Damien Karras emerges from his deep catatonic state, claiming that he has all the answers Kinderman needs.
Nella cittadina, Georgetown,i avvengono strani omicidi legati al mondo religioso. Sono passati ben quindici anni dalla morte di padre Damien Karras, il prete chiamato dai MacNeil per aiutare la loro figlia Regan a liberarsi del demone che la stava possedendo.
